Your Opt-Out Form MUST be returned to the Claims and Noticing Agent so as to be actually received by the Claims and Noticing Agent on or before the Opt-Out Deadline, which is 4:00 p.m. (prevailing Central Time) on March 17, 2023. Opt-out forms sent by facsimile or email will not be accepted. Forms received after the Opt-Out Deadline will not be effective.
